Official Sweepstakes Rules

Eligibility

The Sweepstakes is open to legal residents of the United States who are 18 years of age or older at the time of entry. Employees, officers, directors, agents, and immediate family members of Sponsor, as well as anyone living in the same household, are not eligible to enter. Void where prohibited by law.

Sweepstakes Period

The Sweepstakes begins on June 1, 2026 and ends on August 30, 2026 at 11:59 PM Eastern Time ("Promotion Period"). Entries received after the Promotion Period will not be eligible.

How to Enter

During the Promotion Period, complete the official online entry form available at creationsmedspa.com.

Limit one (1) entry per person unless otherwise stated. Duplicate, incomplete, fraudulent, automated, or mechanically generated entries are void.

Prize

One (1) winner will receive a $2,500 Creations Med Aesthetics & Wellness Spa Gift Card (Approximate Retail Value: $2,500).

The Gift Card may be redeemed only toward eligible services offered by Creations Med Aesthetics & Wellness Spa. The prize is non-transferable, may not be redeemed for cash except where required by law, and may not be substituted except at Sponsor's sole discretion, who reserves the right to substitute a prize of equal or greater value if necessary.

Winner Selection and Notification

One winner will be selected in a random drawing from all eligible entries received during the Promotion Period. The drawing will take place on or about August 31, 2026. Odds of winning depend on the total number of eligible entries received.

The potential winner will be contacted using the phone number and/or email address provided during registration. If the selected winner cannot be contacted, fails to respond within seventy-two (72) hours, is determined to be ineligible, or declines the prize, Sponsor may select an alternate winner.

Taxes

Winner is solely responsible for any applicable federal, state, or local taxes associated with acceptance or use of the prize.

Publicity Release

Except where prohibited by law, acceptance of the prize constitutes permission for Sponsor to use the winner's name, likeness, city, state, photograph, and testimonial for advertising and promotional purposes without additional compensation.

General Conditions

Sponsor reserves the right to verify eligibility, disqualify any entrant who tampers with the entry process or violates these Official Rules, and cancel, suspend, or modify the Sweepstakes if fraud, technical failures, or circumstances beyond Sponsor's reasonable control affect the integrity or proper administration of the Sweepstakes.

Sponsor's decisions regarding all aspects of the Sweepstakes are final and binding.

Limitation of Liability

By entering, participants agree to release and hold harmless Creations Med Aesthetics & Wellness Spa, its owners, employees, affiliates, agents, and representatives from any liability arising from participation in the Sweepstakes or acceptance or use of the prize.

Sponsor is not responsible for lost, late, incomplete, delayed, misdirected, or technically malfunctioning entries or communications.

Privacy

Information collected in connection with this Sweepstakes will be used in accordance with Sponsor's Privacy Policy.

Winner's List

To request the name of the winner, send a written request within sixty (60) days following the drawing to the Sponsor address below. The winner's first name and last initial may also be announced on Sponsor's website and social media channels.

Governing Law

This Sweepstakes is governed by the laws of the State of Florida.
